1. Who We Are and What We Do

Silver Maple Retirement Strategies LLC is a licensed insurance agency in Wisconsin and an independent retirement planning practice. We work with the following organizations to deliver our services:

• American Senior Benefits (ASB) for insurance products and Medicare-related work
• Gradient Advisors, LLC, an SEC Registered Investment Advisor, for investment advisory services
• Integrated Trust Systems (ITS) for estate planning document delivery through their eStatePlan platform

The Site provides general information about our practice and our planning framework (The Four Branches), and offers ways for prospective clients to learn more, take our Your Retirement Snapshot survey, or get in touch.

2. Use of the Site

You may use the Site for lawful, personal, non-commercial purposes only. You agree that you will not:

• Use the Site in any way that violates applicable law or regulation
• Use the Site to transmit harmful code, viruses, or interfere with the Site's operation
• Attempt to gain unauthorized access to any part of the Site or our systems
• Use any automated system (bots, scrapers) to access the Site without our prior written consent
• Use the Site to harass, threaten, impersonate, or defraud any person
• Submit information that is false, misleading, or that you do not have the right to submit
• Use the Site to solicit business from our visitors or compete with our practice

We reserve the right to deny access to the Site to any person who violates these Terms.

3. Eligibility and Service Area

The services described on this Site are intended for residents of the State of Wisconsin. Our Your Retirement Snapshot survey enforces a Wisconsin-only ZIP code requirement. If you are not a Wisconsin resident, we may not be able to serve you.

You must be at least 18 years old to submit any form on the Site.

4. The Information on This Site Is Educational, Not Advice

Specifically:

• Not legal advice. Nothing on this Site is legal advice. For legal questions, including questions about wills, trusts, powers of attorney, or estate planning documents, consult a licensed attorney in your jurisdiction.

• Not personalized financial advice. General information about retirement planning concepts on this Site is not personalized financial advice. Personalized advice requires a complete review of your individual circumstances, which only happens after we begin a formal engagement.
• Not personalized insurance recommendations. General information about insurance concepts on this Site is not a recommendation that you purchase any specific insurance product.
• Not an offer or solicitation of securities. Nothing on this Site is an offer to sell or a solicitation to buy any security. Investment advisory services are offered only through Gradient Advisors, LLC pursuant to a written advisory agreement.

You should not act, or refrain from acting, based solely on information found on the Site. Always consult appropriately licensed professionals before making decisions about your retirement, insurance, taxes, investments, or estate plan.

5. Insurance Solicitation Disclosure

This is a solicitation for insurance. Information on this Site, including any forms you may submit, is for the purpose of marketing insurance products. By engaging with the Site, including by submitting any form or calling any number listed on the Site, you may be contacted by a licensed insurance agent.

When you call any phone number listed on this Site, you will reach a licensed insurance agent. We are not affiliated with or endorsed by the United States government or the federal Medicare program.

We conduct our planning, sales, and enrollment conversations in person, not over the phone. Phone calls are used for scheduling, appointment confirmation, follow-up to a meeting, and general questions. If you have a sales or product-specific question, we will schedule an in-person meeting with you to discuss it properly.

We do not offer every plan available in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Assistance Program (SHIP) to get information on all of your options.

6. Forms and Communications

When you submit a form on the Site (including Your Retirement Snapshot or our Contact form), you represent that the information you provide is accurate and that you have the right to submit it.

6.1 Contact form

By providing your phone number and checking the consent box on our Contact form, you consent to receive non-marketing text messages from us about scheduling, appointment confirmations, and follow-up related to your inquiry.

6.2 Your Retirement Snapshot and other lead-generation forms

By providing your phone number and checking the consent box on Your Retirement Snapshot, you grant permission for a licensed insurance agent affiliated with Silver Maple Retirement Strategies LLC to contact you by phone, text, or email regarding your Medicare options, including Medicare Supplement Insurance, Medicare Advantage, and Medicare Part D Prescription Drug Plans, and other retirement planning services. This permission expires twelve (12) months from the date you provide it.

When you submit Your Retirement Snapshot, your information is held only by Silver Maple Retirement Strategies LLC. We do not share your Snapshot information with any other organization, agent, or lead vendor at the Snapshot stage. Information sharing with affiliated organizations (Gradient Advisors, ITS, or insurance carriers) happens only after you have signed a separate engagement agreement authorizing that sharing for the specific service. 6.3 Common SMS terms

• Reply STOP at any time to opt out of text messages
• Reply HELP for assistance
• Message and data rates may apply
• Consent to text messages is not a condition of any service

6.4 No representation about specific plan benefits

Nothing on this Site is intended as a marketing communication for any specific Medicare Advantage, Medicare Part D, or Medicare Supplement plan, premium, benefit, copay, or carrier. For information about specific plans available in your area, please call to speak with a licensed insurance agent.

6.5 CMS one-to-one consent rule

The Centers for Medicare & Medicaid Services (CMS) requires that, before a Third-Party Marketing Organization (TPMO) shares Medicare beneficiary data with another TPMO, the beneficiary must give prior express written consent that names the specific recipient. We comply with this rule by not sharing Snapshot data with any other TPMO at the Snapshot stage. If our practices change in the future, we will update our consent forms to comply with this rule and provide you a clear opportunity to opt in or decline each named recipient.

7. Third-Party Tools and Oversight

7.1 Third-party tools

The Site uses several third-party tools to function, including GoHighLevel (hosting, forms, CRM), Calendly (appointment scheduling), and Google Analytics 4 (traffic analytics), as well as those listed in our Privacy Policy. When you use these tools through the Site, you may also be subject to those vendors' own terms and privacy practices.

7.5 Carrier, ASB, and CMS oversight

Some content on this Site, particularly Medicare-related content, is subject to oversight by the Centers for Medicare & Medicaid Services (CMS), American Senior Benefits (ASB), the carriers whose products we represent, and the Wisconsin Office of the Commissioner of Insurance.

We make commercially reasonable efforts to keep this Site compliant with applicable carrier, CMS, and state requirements. If you spot information that appears incorrect or outdated, please contact us using the information at the bottom of this page.

7.6 Language assistance and auxiliary aids

Language assistance services and auxiliary aids and services are available to you free of charge. If you need help understanding the Site, our forms, or any communication from us in a language other than English, or if you need accommodation for a disability, please contact us and we will work with you to provide assistance.

8. Intellectual Property

The Site and its original content — including text, graphics, logos, the Four Branches mark, the maple tree visual identity, page designs, and the Your Retirement Snapshot survey questions and structure — are the property of Silver Maple Retirement Strategies LLC and are protected by United States copyright, trademark, and other intellectual property laws.

You may view and print pages from the Site for your personal, non-commercial use. You may not copy, reproduce, republish, or redistribute any part of the Site for commercial purposes; modify or create derivative works based on the Site; use any of our names, logos, marks, or branding without our prior written permission; or frame or mirror any part of the Site without our prior written permission.

9. Links to Other Sites

The Site may contain links to third-party websites — including carrier websites, government resources (such as Medicare.gov and the Social Security Administration), professional organizations, or partner sites. These links are provided for convenience only.

We do not control, endorse, or assume responsibility for the content, privacy practices, or terms of any third-party site. Visiting a third-party site is at your own risk.

10. No Warranties

THE SITE AND ALL CONTENT ON IT ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED.

To the fullest extent permitted by law, we disclaim all warranties, including without limitation implied warranties of merchantability, fitness for a particular purpose, and non-infringement; warranties that the Site will be uninterrupted, timely, secure, or error-free; warranties that information on the Site will be accurate, complete, current, or reliable; and warranties regarding the results that may be obtained from your use of the Site.

You assume all risk associated with your use of the Site.

11. Limitation of Liability

To the fullest extent permitted by Wisconsin law, in no event will Silver Maple Retirement Strategies LLC, its members, managers, officers, employees, agents, contractors, or affiliated organizations be liable to you or any third party for any indirect, incidental, consequential, special, exemplary, or punitive damages; any loss of profits, revenue, data, or business opportunity; any damages arising from your use of, or inability to use, the Site; or any damages arising from any reliance on information found on the Site.

This limitation applies regardless of the legal theory (contract, tort, negligence, strict liability, or otherwise), even if we have been advised of the possibility of such damages.

In any event, our total aggregate liability arising out of or relating to your use of the Site is limited to one hundred dollars ($100.00) or the amount you have paid to us in the twelve months preceding the claim, whichever is greater.

Some states do not allow the exclusion or limitation of certain damages, so some of the above limitations may not apply to you. Nothing in these Terms limits any liability that cannot be limited under applicable law.

12. Indemnification

You agree to defend, indemnify, and hold harmless Silver Maple Retirement Strategies LLC, its members, managers, officers, employees, agents, contractors, and affiliated organizations from and against any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorney's fees) arising out of or relating to your use of the Site; your violation of these Terms; your violation of any rights of any third party; or information you submit through the Site that is false, misleading, infringing, or unlawful.

13. Governing Law and Venue

These Terms are governed by the laws of the State of Wisconsin, without regard to its conflict-of-law principles.

Any dispute arising out of or relating to these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Juneau County, Wisconsin (or the federal district covering Juneau County). You consent to the personal jurisdiction of those courts and waive any objection based on inconvenient forum.

14. Class Action Waiver

To the fullest extent permitted by law, any claim or dispute you bring against us must be brought in your individual capacity, and not as a plaintiff or class member in any purported class action, collective action, or representative proceeding. A court may not consolidate more than one person's claims or otherwise preside over any form of representative or class proceeding.

If this class action waiver is found to be unenforceable as to any particular claim, then that claim will be severed and proceed in court, and the remainder of these Terms will continue to apply.

15. Changes to These Terms

We may update these Terms from time to time. When we do, we will update the "Last Updated" date at the top of this page. Material changes will be communicated through a notice on the Site or, where appropriate, by email. Your continued use of the Site after changes are posted constitutes your acceptance of the updated Terms.

16. Termination

We may, at our discretion and without notice, suspend or terminate your access to the Site if we believe you have violated these Terms or applicable law. Sections that by their nature should survive termination — including Sections 4, 8, 10, 11, 12, 13, and 14 — will survive any termination.

17. Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that the remaining Terms continue in full force and effect.

18. Entire Agreement

These Terms, together with our Privacy Policy and any other notices posted on the Site, constitute the entire agreement between you and Silver Maple Retirement Strategies LLC regarding your use of the Site.
